ESSENTIAL FUNCTIONS OF THE POSITION:

The primary contact for participants in the Brother’s United Program, the Case Manager will guide the participant through the program and provide continued support in the initial stage of the program and throughout the program.  The Case Manager will manage an active caseload of participants. 

The Case Manager will assume the primary duties related to intake into the Brothers United program. This may include but not limited to:

  • Provides support in the initial stages of the program and throughout the program.
  • Completes phone call log with full details.
  • Responsible for recruitment of participants.
  • Follows up with participants ensuring completion of the program.
  • Completes Case Management services for participants, according to the program model.
  • Manages an active caseload of participants.
  • Links and refers participants with community resources and service providers.
  • Follows up with participants to ensure contact with service providers.
  • Schedules and conducts weekly case management meetings with participants.
  • Attends weekly staff meetings.
  • Responsible for completing assigned monthly, quarterly and annual reports.
  • Coordinates with staff to develop a community and partner outreach and recruitment plan based on program description.
  • Other duties as assigned.

QUALIFICATIONS:

  • Must be self-motivated, self-driven, and work at a fast pace.
  • Must be able to work independently without daily supervision.
  • Must be able to provide outreach to the community.

  • Must have the ability to coordinate services based on the participants needs.
  • Must be able to perform all task necessary to achieve the organizations mission.
  • Must provide timely intervention for program participants when needed.
  • Must be able to facilitate evidenced based curriculum with the exception of MRE.
  • Must be able to motivate others toward achieving goals.
  • Must be able to maintain participant’s files including case and progress notes.
  • Must be able to maintain data system updates.
  • Must be able to assist with collection, preparation, and submission of weekly and monthly enrollment reports.
  • Must be able to encourage teamwork and provide leadership to program participants.
  • Must be proficient in the use of computers, the internet, and data entry, as well as be knowledgeable of various computer systems: Facebook live, Zoom, Google drive, Google calendar etc.
  • Must have the ability to speak publicly.
  • Must have the ability to handle multiple tasks simultaneously with a high level of competence.
  • Must have excellent organizational skills, problem solving skills, organized, and have good record keeping skills.
  • Must be knowledge of related services for individual support and be able to work with area social service agencies, faith-based organizations, etc.
  • Must have experience in client support practices.
  • Must be able to complete various duties while simultaneously maintaining each program’s processes, rules, and guidelines.
  • Must be patient, even-tempered, flexible and results driven.
  • Must possess a valid driver’s license, have access to an automobile, and provide proof of liability insurance.

Please note: This position is evenings Monday- Friday and weekend hours Saturday and/or Sunday.

EDUCATION:

  • High school/GED required, a Bachelors Degree is preferred or any combination of training and experience equivalent to two (2) years of college. Previous experience working in a Fatherhood program is preferred.
